The Community Support Staff provides direct, intermittent support for people in the SLA/CILA program through counseling, training, teaching, assistance and empowering techniques.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein as such duties are a logical assignment to the position.

  1. Works cooperatively as part of the Community Support Team to provide supports for people in the Family CILA program.
  2. Develops and maintains positive relationships with the people in the Family CILA program and their families.
  3. Completes documentation as required by DHS rules and regulations and LCF policies and procedures.
  4. Serves as an advocate in support of people’s rights and choices.
  5. Assists with scheduling and transportation for medical/dental appointments ensuring follow through with recommendations and reporting effects and side effects of medications.
  6. Assists people in the Family CILA program in accessing transportation.
  7. Provides assistance in money management.
  8. Responsible for direct implementation of all goals outlined in the ISP and data tracking of these goals.
  9. Serves as a mentor for individuals in the Family CILA program.
  10. Participates in all required meetings.
  11. Attends and participates in staff training and development programs.
  12. Is aware of all safety activities in the workplace and immediately reports all accidents and safety hazards to the QIDP.
  13. Maintains annual American Heart Association certification in First Aid and CPR.
  14. Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or her immediate supervisor.

Qualifications

The Community Support Staff must have two years of previous experience working with people with developmental disabilities. They must have a valid driver’s license, proof of insurance and must be able to be insured by Little City Foundation’s auto insurance policy. When utilizing a personal vehicle for transportation of people in the Family CILA program, they must utilize an insured vehicle. The Community Support Staff must be able to speak and write clearly and communicate information effectively in English. The Community Support Staff must successfully complete Little City’s Orientation and Training program and maintain certification in CPR, First Aid, and CPI. Two years of college is preferred.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

While performing the duties of this job, the employee is subject to sitting, walking, and standing for prolonged periods; frequently grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls. The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee is subject to assist and support up to 50 pounds with varying amounts of assistance on a reoccurring basis. The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Must have the ability to perform CPR and CPI. Occupational exposure to blood, body fluids, infectious substances, including communicable diseases when performing personal care (Category III). Occasional exposure to cleaning agents and disinfectants. Residential work environment with a noise level of normal to loud. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
