KEY JOB RESPONSIBILITIES: Provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. Provides service by each patient's home health plan of care and LPNs scope of practice. Maintains compliance with federal and state regulations, reimbursement guidelines as well as agency's policies and procedures.
Additionally, the Licensed Practical Nurse (LPN) will:
- Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
- Demonstrates ability to continually evaluate the patients plan of care and communicate findings with Registered Nurse.
- Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physicians orders.
- Evaluates the significance of assessment findings and communicates any pertinent information about the patient's clinical status and ongoing needs to others in a timely fashion.
- Demonstrates the ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Demonstrates knowledge of medications and their correct administration based on the age of the patient and patients' clinical assessment.
- Demonstrates knowledge of medications and their correct administration, follows the seven (7) medication rights to reduce the potential for medication errors. (Right: patient; medication; dose; route; time; documentation; reason)
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team.
- Performs patient care responsibilities considering needs to the standard of care for patients age.
- Documents wound measurements with each comprehensive assessment and weekly thereafter.
- Performs all aspects of patient care in an environment that optimizes patient safety, quality, and reduces the likelihood of medical/health care errors.
- Help optimize the capital structure and support strategic initiatives.
- One must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
- Always maintains patient confidentiality, including all HIPAA regulations.
- The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patient's home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
