POSITION TITLE:     Case Manager – Transitional Case Management (TCM)

PROGRAM:               Special Services

GENERAL STATEMENT OF RESPONSIBILITIES:

This position provides and coordinates support services and referrals for treatment services.

SUPERVISION: All duties are performed under the supervision of the Program Manager.

EXAMPLES OF RESPONSIBILITIES:

  1. Conducts pre-discharge assessment at correctional facilities as requested by DOC personnel.
  2. Based on screening and assessment, identifies the case management services to be provided and agreed upon by the client.
  3. Develops positive working relationships with supervisor, members of the Homeless Initiative team, other co-workers, primary clinicians and community programs and providers.
  4. Performs case management in the community and home environments.
  5. Develops appropriate therapeutic relationships with clients.
  6. Abides by the Code of Ethics and Federal Laws of Confidentiality.
  7. Assesses for and refers clients to the most appropriate treatment settings.
  8. Learns and utilizes techniques of motivational enhancement therapy to secure client engagement in case management.
  9. Assists the client in securing community living arrangements and housing.
  10. Develops awareness of community resources for housing.
  11. Advocates for clients with landlords to maintain or acquire housing.
  12. Assists the client with applications for benefits and entitlements.
  13. Coordinates and participates in the development of an individual community service plan ensuring the participation of all interested parties.
  14. Transports clients to services consistent with the individual community service plan.
  15. Makes home visits and provides active outreach in the community to maintain client engagement.
  16. Links clients to services such as medical, child care, vocational/educational, etc.
  17. Provides ongoing assessment, intervention, and monitoring including; but not limited to relapse prevention assessment.
  18. Provides assistance with budgeting.
  19. Maintains current progress notes and other relevant documentation on all assigned clients. Submits monthly and other reports/information to the Director in a timely manner.
  20. Provides a minimum 90 day follow-up after clients are discharged from case management

Qualifications

EDUCATION/EXPERIENCE:

Bachelor’s degree or associate degree with two years of experience in the field of substance abuse and dual diagnosis or in providing services to clients in the criminal justice system or selected other experience in the human services field.  Certified Alcohol and Drug Counselor (CADC) preferred.  Experience in case management preferred.  Candidate must be flexible, with a high degree of motivation, exercise good judgment, and be able to work independently.

Candidate must possess a valid driver’s license, be willing to use own car for agency business, clean driving record and  valid auto insurance in compliance with applicable Connecticut laws and regulations.  CPR and First Aid certified.
