JOB DESCRIPTION

DIVISION: Georgia Sheriffs Youth Homes

JOB TITLE: Social Service Coordinator

DEPARTMENT: Child Care

REPORTS TO: Resident Director

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Position serves as the liaison between Youth Homes and other agencies such as DJJ and DFACS by assuring compliance with court appearances, panel reviews, court orders, etc.
  2. Receives calls regarding possible new referrals and screening application for placement
  3. To be informed and knowledgeable of other child care and treatment agencies and programs in this community and across the state.
  4. To arrange and help supervise, when needed, off-campus trips such as visits to sports events, entertainment events, field trips, etc.
  5. To maintain open lines of communication with Houseparents and meet formally to ensure that they are kept informed of pertinent information concerning the residents in their care and to review Individual Care Plans.
  6. Ensures that the Resident Director is kept informed of pertinent information concerning the residents, programs, staff and other agencies involvement.
  7. To assist the Resident Director in coordinating training activities for staff.
  8. Monitors residents health care needs. This includes scheduling annual physicals and 6 month dental cleanings for all residents and follow-up appointments as needed. Provides transportation for these appointments. A record of treatment will be completed and documented according to DHR/ORS requirements. The Social Service Director and Houseparents will be informed to follow any special recommendations.
  9. Coordinates residents visitation requests, including caseworker transportation arrangements and notification to Houseparents.
  10. Assists Resident Director in assuring supervision for residents when their Houseparents are taking vacation.
  11. Ensures that the mental health needs of residents placed in long term care are being met by: recruiting counselor (GA Medicaid recipients), arranging schedules, assuring proper follow-up, and assuring documentation of appointments in respected files.
  12. Monitors residents medication use by ensuring that a physician who has responsibility for the diagnosis and treatment of the childs condition is the prescribing physician, by monitorint the medication logs maintained by Houseparents, and notifying Resident Director of any discrepancy.
  13. Makes sure that the residents social needs are taken care of. This may include, but not limited to filing for work permits, driver tests, special clothing or other needs.
  14. Assists Resident Director in public relations tasks such as providing tours of campus and talking to interested groups about programs.
  15. Maintains an open line of communication with Education Coordinator to determine if his/her service or assistance is needed.
  16. To be trained by the school system and serve as a surrogate parent for children who have Individual Education Plans (IEPs) as related to their educational goals. Signs all pertinent educational forms.
  17. Assists with fundraising activities when requested by Resident Director.
  18. Performs other duties related to the child care operation and management as assigned by the Resident Director.

Qualifications

EDUCATIONAL REQUIREMENTS:

  1. A bachelors degree from an accredited college or university in social work, psychology, childhood education, educational counseling and psychology, or a related field is required. In addition, applicants must have either two years of experience in the field of child care or be supervised by another Human Services Professional with a masters degree in one of the above discipline; or
  2. Possess a masters degree from an accredited college or university in one of the above disciplines.

Must have a current drivers license and an excellent driving record.
