SUMMARY: The CAPABLE/GAP Medical Assistant will collaborate with older adults to deliver personal support components of the CAPABLE program after completion of required CAPABLE training through Johns Hopkins University School of Nursing. CAPABLE is a home-based program to address functional opportunities and promote independence and tenure in the community with older adults. The GAP program is a home- based transitional program that helps chronically- ill older adults obtain Long Term Care Medicaid funded services, while helping to support their immediate needs. The Medical Assistant will work with the CAPABLE OT (occupational therapist) and CAPABLE RN (registered nurse) to assist with any personal care support related to the CAPABLE model.  The Medical Assistant is responsible for assisting OT Program Manager in ensuring all metrics and deliverables are met based on funding requirements.  Under the GAP program, the Medical Assistant will assist with ensuring that clients are continuing to successfully with their CAPABLE goals 30-90 days post completion of the program.

Qualifications

QUALIFICATIONS: High school diploma or equivalent required. Completion of a Medical Assistant training program or certification from a recognized institution. Certification from a recognized medical assistant certification board (e.g., AAMA, AMT) preferred. BLS (Basic Life Support) certification required. A minimum total of two years of working experience with frail or elderly population. Requires valid driver’s license. Preferred Qualifications: A minimum total of two years of working experience with frail or elderly population.

 

PHYSICAL DEMANDS: Position requires moderate visual concentration for patient charting/documentation. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The MA requires the mobility to ambulate or lift a patient with or without the assistance of devices more than 75 pounds using proper body mechanics.

 

WORK ENVIRONMENT: A part-time position works a 15–20-hour work week with occasional requests to work additional hours as needed.  The MA position may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). This position encounters stressful situations with patients and their families regarding end-of-life care. The MA must be able to accurately, timely and concisely document the patient/family encounter using a computer. The MA is required to perform patient visits utilizing their personal vehicle. Although each MA may be designated to a specific department/region in our service area, at times they may be required to serve patients outside of their department/specific region.
