QUALIFICATIONS: Graduate of an accredited school of Nursing. Licensed Practical Nurse (LPN) license required. Experience in hospice or home health preferred. Knowledge of Joint Commission standards and hospice-specific regulations preferred. CPR certification required. Valid driver’s license, automobile insurance, and reliable transportation required. Strong communication, leadership, customer service, and critical thinking skills required. Proficiency in Microsoft Office and database systems required.

PHYSICAL DEMANDS: Position requires moderate to intense visual concentration, reading, using a computer and reviewing patient charts. This position has daily contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers and HSI staff requiring excellent communications and problem-solving skills and professional demeanor. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The Clinical Services Specialist requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.

WORK ENVIRONMENT: This position works a 30-40 hour workweek, to include rotating days, evenings, or weekends and occasional requests to work additional days/hours/holidays as needed. This position may require some commuting between the Hospice Savannah facilities utilizing a personal vehicle. This position encounters stressful situations with patients and their families regarding end of life care. The Clinical Services Specialist may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). The Clinical Services Specialist must be able to accurately, timely, and concisely document the patient/family encounter using a computer.
