JOB SUMMARY

 

The oncology nurse navigator (ONN) optimizes the patient experience and improves outcomes and enhances patient satisfaction by providing seamless patient centered navigation through the cancer care continuum from initial diagnosis until transition into survivorship or end of life care. Navigators provide high quality, timely care through collaboration with physicians and other members of the interdisciplinary care team to reduce real and/or perceived barriers to receiving their needed care. The navigator facilitates care-coordination with referrals to specialists, appointment scheduling, and medical record review and ensures that patients have access to social services, nutritional support, financial counseling, clinical trials, genetic counseling, education and resources at Hamilton and in the surrounding community. The ONN serves as a patients point of contact throughout the cancer care continuum, consistently promoting patient advocacy, addressing patient/family concerns, and assuring high quality care.

Qualifications

JOB QUALIFICATIONS

Qualifications:

Education: ASN required. Bachelors degree in Nursing or Management preferred.

 

Licensure: Licensed as a Registered Nurse in the State of Georgia. BLS required

 

Experience: Minimum of 3 years of clinical nursing experience, preferably oncology. Outpatient clinical experience preferred.

 

Certification: OCN, AOCN, AOCNS certification preferred or OCN to be obtained within 24 months. Nurse navigation certification to be obtained as soon as certification will allow based upon amount of patient experience. ONS/ONCC Chemotherapy and Biotherapy competent or completed within one year of hire.

  7. Ability to work within a value-based care framework, identifying the most appropriate resources to help the patient receive seamless care
  8. Basic computer skills- Microsoft Word, Excel, and database required to assess performance of program
