Case Manager - Mental Health Hospital

Full-Time / Monday - Friday

Gateways Hospital and Mental Health Center is seeking a motivated individual to fulfill the role of Case Manager in the Statewide Transitional Residential Program (STRP). The STRP is a 17-bed (12 male, 5 female), unlocked, residential, forensic program, and is the highest level of care with the CONREP system (CAs Conditional Release Program for individuals transitioning from the Department of State Hospitals back into the community). Clients are referred from county CONREP programs throughout the state and the STRP program serves as a transitional, 90-day, step-down program between the state hospital and the community.

SUMMARY OF POSITION:

As part of the interdisciplinary STRP treatment team, the Case Manager is responsible for guiding clients in the re-integration into the community through the establishment of benefits (Social Security, Veterans Administration, Medi-Cal, ACCESS, etc.) and assistance in accessing outside medical, dental, financial, housing, transportation, clothing, retail, and immigration resources.

The Case Manager will also oversee the structured day program activities, including skill building groups, community outings, recovery groups, and leisure time. The Case Manager monitors clients behavior and functioning, assists clients in developing skills to support their community integration, and collaborates with the treatment team and residential staff to ensure quality care for all clients.

DUTIES AND RESPONSIBILITIES

  • Responsible for securing all benefits for clients through liaison with Social Security, Medi-Cal, Veterans Administration or other benefits agency. Keeps the Program Coordinator informed about clients benefit status.
  • Assists clients in obtaining disabled bus passes, California I.D., immigration documentation, and other services as needed.
  • Coordinates with the LVN and the entire treatment team to arrange for assisting clients in accessing medical and dental care.
  • Transports and accompanies clients to and from appointments as determined necessary.
  • Oversees Socialization Improvement Program (SIP), which includes, but is not limited to, skill building groups, outings, and leisure activities.
  • Understands billing codes and requirements, including billing and documentation requirements for case management services and socialization improvement groups.
  • Documents all services provided within program guidelines and expectations in a timely manner.
  • Provides the treatment team with ongoing updates on clients functioning, behavior, compliance, progress, and engagement.
  • Completes and documents all orientation and discharge services within expected timeframes.
  • Assists in the collection of urine specimens for drug testing when staffs responsible are not available. Inputs documentation into the record in a timely manner.
  • Conducts random checks of premises and buildings, including client room searches.
  • Assists in situational crisis intervention as needed.
  • Reports safety hazards, maintenance and housekeeping needs to the Residential Manager.
  • Performs other additional tasks as directed.

 

Qualifications

EXPERIENCE/QUALIFICATIONS:

  • Prior paraprofessional counseling experience.
  • Academic training in Behavioral Sciences.
  • Experience with the mentally ill and/or forensic populations.
  • California Class B Drivers License
  • Driving record acceptable for coverage by Gateways insurance carrier
  • Fire and Safety Training*
  • First Aid Training Certification*
  • CPR Certification*
  • Management Of Aggressive Behavior*(M.O.A.B.)

* Note: If indicated certifications/licenses are lacking, they must be obtained within 180 days of hire.

EDUCATION: Minimum Education Required:

Bachelors Degree in Behavioral Sciences or related area.

Desired Education Masters Degree in Behavioral Sciences or related area.

REQUIREMENTS

  • Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance.
  • Valid California Drivers license
  • TB clearance

  • Approximately 50% of time is spent sitting, while frequently required to walk, stand and bend. Occasionally required to stoop, kneel, crouch or crawl. Employee will be required to lift and/or move unassisted up to 25 pounds.
