Full-Time Care Manager

35 hours per week M-F

 

 

SeniorCare Inc. has a full time Care Manager position to provide client-centered assessments and then authorizes, coordinates and monitors services provided to elders to assist them in living independently in their community.

We are seeking an individual with a bachelor’s degree in social work or related field. Effective communication skills are necessary for interaction with the client, family, and other supports. Must be able to utilize a computer for data entry, word processing and to research resources. Experience with elders and/or in a community service setting is preferred.

Qualifications

EXPERIENCE AND REQUIRED SKILLS:

 

Licensure:  Commonwealth of Massachusetts Licensure in Social Work at the LSW level, or eligible, or licensure pending; or commitment to obtain said licensure within a reasonable time frame.  If not licensed upon hiring, agrees to attend bi-weekly licensure supervision until licensed.

 

Continuing Education:  Will be maintained by Care Managers in accordance with their licensure requirements.

 

Experience working with older people and/or experience working in a community service setting is preferred.

 

Skill/Knowledge:  Ability to assess needs and problems, and to develop appropriate, individualized, goal-oriented service plans.

 

Intermediate Computer Skills. Must be able to utilize a computer for data entry, word processing, and to research resources.

 

Ability to work independently.

 

Ability to establish an effective consumer/case manager relationship.

 

Effective communication skills necessary for appropriate interaction with consumer, family and other formal and informal care systems.

 

Awareness of the aging process, and of dependency issues and their effect on consumers, as well as a respect for the consumers’ right to self-determination.      

 

PHYSICAL DEMANDS AND WORKING CONDITIONS: 

PHYSICAL DEMANDS:

 

            Lifting & Carrying at a light level (up to 10 pounds).

            Pushing & Pulling at a light level (up to 10 pounds).

            Stooping & Crouching on a frequent basis.

            Reaching the hands and arms in any direction.

Seeing:  acuity, depth perception, and field of vision are such that one can identify, judge, and react to objects or the environment.

Oral & Graphic Expression of the English language in a clear and effective manner.

ENVIRONMENTAL DEMANDS:

 

            Personal Interpretation of feelings as they relate to death, dying, disabilities, aging, illness, loss, or change.

 

EDUCATION:

            Bachelor’s degree in social work, human services, nursing or a related field.

Bachelor’s degree in social work, human services, nursing or a related field per EOEA Business Rules dated August 25, 2006.

 

 

RELATIONSHIPS:

 

Internal Contacts: Clinical Assessment and Eligibility RNs, Home Care Supervisors, Special Programs Director, Care Managers, Protective Service workers and Supervisors, Age Info staff, Director of Home Care, and other agency program staff as indicated.

                                                                       

External Contacts: Consumer informal supports, physicians, nurses and therapists from home health agencies, vendor agency staff, hospice providers, nutritionists, housing managers, mental health professionals, and other home and health care professionals.

 

Reports to:

 Home Care Supervisor or the Special Programs Director.

 

Directly supervises:  N/A
