Role OverviewWe are seeking a dedicated and autonomous Home-Based Primary Care Nurse Practitioner to join our growing team. In this role, you’ll conduct in-home visits, manage chronic and acute conditions, and build meaningful relationships with patients and their families. You’ll be part of a supportive, mission-driven practice that values flexibility, independence, and clinical excellence.

Key Responsibilities

• Provide comprehensive primary care services during in-home visits
• Perform physical exams, diagnostic assessments, and routine screenings
• Manage chronic conditions and acute illnesses
• Prescribe medications and develop individualized care plans
• Coordinate with other healthcare providers and community resources
• Document visits and maintain accurate medical records
• Educate patients and caregivers on health management and preventive care

Qualifications

• Licensed Nurse Practitioner in the state of New Jersey
• Board-certified in Family or Adult-Gerontology Primary Care
• Minimum of 1 year of clinical experience (home care experience preferred)
• Strong clinical judgment and ability to work independently
• Excellent communication and interpersonal skills
• Valid driver’s license and reliable transportation
