Position Summary:
The Family Navigator Lead helps families achieve stability by providing guidance, resources, and emotional support. They collaborate with parents, foster families, and case managers to ease transitions and strengthen family bonds. Responsibilities include assisting with case management, tracking referrals, and coordinating services. This role also advocates for families, ensuring they receive the support they need.
Essential Job Functions:
- Serve as primary point of contact for the Peer Navigator team.
- Collaborate with the Clinical Manager to oversee the work of the Peer Navigators and provide guidance and support.
- Act as a peer leader, setting a positive example and providing guidance for others to follow.
- Support the well-being, independence, and self-sufficiency of clients while guiding them through case plans to encourage successful reunification.
- Collaborate with parents, foster parents, and case managers to create a smooth and supportive transition plan for children returning home.
- Assist with training team members and provide support as needed.
- Foster positive relationships with parents to strengthen family bonds, enhance parent-child connections, and encourage engagement in case planning and services.
- Offer guidance, education, and supportive services tailored to the needs outlined in each service plan.
- Work alongside case managers, counselors, and social workers to provide case management support, including information sharing, assessments, referrals, and connections to resources.
- Track referrals and service linkages to ensure consistency and continuity of care.
- Encourage and empower clients by sharing experiences, offering emotional support, and helping them access the services and tools they need to succeed.
- Participate in weekly staff meetings to discuss client progress and provide input as requested.
- Support the team and organization with additional responsibilities as needed.
- Advocate for and support children and families in a caring and respectful manner.
- Serve as a family/individual representative and advocate in program/agency activities.

· Provide Peer support to individuals/families by using personal and professional life experiences.
