POSITION SUMMARY:

The individual in this position will serve as the primary point of contact for individuals and families experiencing homelessness in Madera County. They will accept referrals through the Coordinated Entry System (CES) and be responsible for conducting screenings, identifying housing solutions, coordinating transportation, and providing temporary financial assistance and service linkages to support rapid housing stabilization. This includes arranging, coordinating, monitoring, and delivering services that meet clients’ housing needs and connect them to community resources to achieve long-term housing stability. Essential Job Functions include those listed below.

ESSENTIAL JOB FUNCTIONS:

  • Accept referrals through the Coordinated Entry System (CES) and from approved community partners, walk-ins, and collaborative agencies.
  • Conduct standardized screenings for newly homeless individuals and families.
  • Conduct intake and assessment of incoming clients to determine program eligibility and service needs.
  • Provide case management services to individuals and families through Rapid Re-Housing and Diversion strategies.
  • Meet with clients at least weekly to support progress toward individualized housing and service plans.
  • Immediately address safety concerns, including prompt connection to domestic violence providers as needed.
  • Link clients to supportive services such as mental health care, substance use treatment, healthcare, childcare, and employment services. Make referrals and schedule appointments with appropriate agencies or service providers.
  • Offer transportation assistance to support clients in resolving their housing crisis.
  • Assist with retrieval of vital documents to obtain housing, unit identification and housing applications
  • Build and maintain relationships with property owners and landlords to secure housing units.
  • Maintain up-to-date and accurate client files in accordance with agency policies and contractual obligations.
  • Ensure timely and accurate data entry in the Homeless Management Information System (HMIS) and the Clinical Data System (CDS)
  • Provide daily, weekly, and monthly reports as required.
  • Assist in data collection, statistical reporting, and completion of Quality Improvement reports.
  • Represent the agency at community meetings and maintain relationships with referring/supporting agencies.
  • Coordinate services across multiple agencies and attend relevant meetings and presentations.
  • Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
  • Perform all other duties as assigned.

Qualifications

ESSENTIAL QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Experience and Competencies:

  • Excellent communication and public speaking skills.
  • Demonstrated ability to work with the underserved populations.
  • Ability to conduct outreach, including street outreach.
  • Good computer skills.
  • Good documentation skills.
  • Be able to multi-task and make independent decisions; and
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
  • Professional appearance and demeanor.
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.
  • Ability to obtain and maintain a criminal history background check.
  • Must be able to obtain and maintain eligibility for WestCare’s Vehicle Insurance.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

EDUCATION and/or EXPERIENCE:

  • Two (2) years case management is preferred.
  • Graduation with a two-year degree in Psychology, Social Work, Community and Human Services or a related field is preferred; and
  • High School diploma or equivalent is required.

LICENSURE/CERTIFICATION:

  • Not applicable for this position.

WORKING CONDITIONS:

  • Work is primarily performed in an office setting.
  • Some travel is required; and
  • Some outdoor activities may be required.
