Position Summary:

Person in this position will be responsible for providing intensive, person-centered case management services to veterans and service members who are identified as at-risk for suicide. This role focuses on addressing immediate needs, reducing barriers to care, and coordinating services to support stabilization, recovery, and long-term wellness. The Case Manager works closely with the intervention and peer support teams to ensure participants receive timely and appropriate resources, including mental health care, substance use treatment, housing, employment support, and other wraparound services. Essential Job Functions include but not limited to those listed below.

Essential Job Functions:

  • Conduct comprehensive assessments to identify each participant’s strengths, needs, and risks.
  • Develop individualized service plans focused on reducing suicide risk and enhancing protective factors.
  • Provide ongoing case management, including coordination of mental health, substance use, housing, and other social services.
  • Support crisis intervention efforts in collaboration with intervention specialists and peer support staff.
  • Conduct regular follow-up with participants to monitor progress, adjust care plans, and ensure service continuity.
  • Serve as a liaison between participants and service providers, ensuring warm handoffs and effective communication.
  • Maintain accurate and timely documentation in compliance with grant requirements and confidentiality standards.
  • Participate in multidisciplinary case conferences, trainings, and team meetings.
  • Assist with tracking grant outcomes and preparing the required reports.
  • Maintain knowledge of local resources, VA services, and suicide prevention str Must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
  • Other relevant duties as assigned.

Qualifications

Essential Qualifications:

To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

 

  • Experience working with high-risk populations, including individuals with mental health or substance use disorders.
  • Knowledge of suicide prevention principles, trauma-informed care, and veteran-specific challenges.
  • Strong interpersonal, organizational, and advocacy skills.
  • Ability to work independently in a fast-paced environment and respond effectively to crisis situations.
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
  • Professional appearance and demeanor.
  • Veteran/Military is preferred.
  • Must be able to successfully obtain and maintain a criminal background clearance
  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required; and
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

Education:

  • Minimum of 2 years of experience in case management, behavioral health, or veteran services.
  • Bachelor’s degree in social work, psychology, human services, or related field is preferred.
  • High school diploma or equivalent required.
