Our services are rooted in compassion, science, and community—and our Case Managers are vital to bridging the gap between clinical care and the real-life support systems that patients need.

About the Role:

As a Case Manager, you’ll work directly with residential patients to assess needs, remove barriers to care, and establish strong aftercare plans. You’ll be the connector between treatment and the community—helping patients navigate housing, employment, transportation, public benefits, legal support, and recovery resources. This role is ideal for someone who values advocacy, problem-solving, and empowering others.

What You’ll Do:

  • Collaborate with clinical and medical teams to support patient treatment goals
  • Conduct non-clinical assessments to identify recovery barriers related to housing, food, employment, and more
  • Use motivational interviewing to develop patient-centered action plans
  • Attend care coordination meetings and monitor patient progress
  • Act as liaison with community partners, healthcare providers, and legal supports
  • Assist patients in accessing public benefits such as SNAP, OHP, SSI, and ID documentation
  • Maintain up-to-date knowledge of community resources and recovery services
  • Develop and maintain personalized service and discharge plans
  • Coordinate care transitions into outpatient, sober living, or higher-level care settings
  • Facilitate telehealth, dental, mental health, and medical appointments as needed
  • Ensure all patient interactions are accurately documented in the electronic health record
  • Travel between sites as needed and attend community events and resource fairs

Qualifications

What You Will Bring:

  • Required: High school diploma or GED
  • Preferred: At least 1 year of experience in case management, peer support, SUD services, or social services
  • Familiarity with Social Determinants of Health and trauma-informed care
  • Strong interpersonal, advocacy, and communication skills
  • Ability to maintain professional boundaries while providing high-touch, compassionate support
  • Cultural humility and sensitivity in working with diverse patient populations
  • Proficiency in Microsoft Office and electronic medical record systems
  • A valid Oregon or Washington driver’s license

Required Certifications (or ability to obtain):

  • Peer Support Specialist (THW) certification within 30 days of hire
  • Basic Life Support (BLS) certification within 90 days
  • Crisis prevention training (e.g., CPI) within 90 days
