POSITION OVERVIEW:
Under the supervision of the Clinical Supervisor, the Licensed Vocational Nurse (LVN) will be assigned to the Full Service Partnership (FSP) program, providing nursing care and supportive services to adults with behavioral health challenges or co-occurring disorders. The Licensed Vocational Nurse is primarily responsible for performing and monitoring key health indicators, medication management, and taking vital signs of clients who meet criteria for services. The Licensed Vocational Nurse will work closely with individuals experiencing behavioral health challenges by supporting them in remaining in their community and reducing the need for residential or inpatient placements. This role requires a commitment to providing high-quality, individualized care to individuals in a manner that respects their unique cultural backgrounds and promotes overall well-being. The Licensed Vocational Nurse will meet deliverable expectations and requirements of the program. Essential Job Functions and Responsibilities:
- Conduct assessments, planning, implementation and evaluation of patient care
- Recognize, report, and record any change in condition or behavior of the patient.
- Accurately communicate in writing through utilization of appropriate charting methods (Electronic Health Record), the patient care summary and proper transcription of medical orders.
- Establish therapeutic rapport with patients, families, and walk-in community members, while respecting patient's rights to privacy and confidentiality of information.
- Communicate pertinent information regarding patients with the team
- Place follow up phone calls to communicate test results and/or discrepancies, as needed
- Provide patient information as necessary
- Complete and document medication count as required and necessary.
- Monitor patient’s post-administration for adverse effects and provide treatment and facilitate emergency response as needed
- Prepare and perform blood pressure screenings
- Weight Assessment and Counseling for Nutrition and Physical Activity
- Take, monitor, and control High Blood Pressure for clients
- Provide patient education and supporting materials.
- Will also provide training and psychiatric education to adults and families, but also stakeholders including internal staff, community partners, and similar as directed; as well as conduct outreach activities to increase knowledge of and engagement in services either for existing clients or as a standalone treatment
- Will participate in program planning and utilization review and quality assurance, and will follow Drug Medi-Cal Organized Delivery System charting and documentation requirements and Agency regulations.
- Demonstrate understanding of emergency situations and response plans.
- Answer patient questions and refer to Providers as necessary
- Following all site equipment storage and security protocols
- Communicate with site leaders regarding supply needs
- Support facility management and daily maintenance routines, including cleanliness and sanitation protocols
- Prioritize patient and staff safety at all times, following protocols and procedures as communicated by site leadership.

- Attend trainings and participate in events relevant to key responsibilities
