POSITION OVERVIEW

Under the supervision of the Clinical Supervisor, the Promotor(a) is assigned to the Stan Connect program which is embedded in the communities of West Modesto to assist the team and provide community members with needed supportive services. The Promotor(a)will participate in community events to increase awareness about services and resources.  The Promotor(a) will use best practices to build trust, rapport, and empower community members. The Promotor(a) will collaborate with partners to ensure community members receive appropriate services. The Promotor(a) will meet deliverable expectations and requirements of the program. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture.  

 

Essential Job Functions and Responsibilities: 

  • Embedded in the community to assist with referrals, screenings, assessments, and provide case management services.
  • Use cultural humility and best practices to engage community members.   
  • Assist with navigating and accessing services.
  • Assist with educational and support groups. 
  • Assist with workshops and wellness events. 
  • Participate in walkabouts in the community to build rapport and increase awareness about services and resources. 
  • Meet with individual and/or family members to provide additional support, or referrals to services as needed. 
  • Collaborate with trusted partners to advocate and link community members to appropriate services.
  • Performs other related duties as assigned.

Documentation and Compliance

  • Complies with all agency’s policies and procedures and maintains strict HIPPA guidelines.
  • Submits records of service information and tracking consistent with program policies.
  • Maintains records, prepares reports and performs other administrative duties as required.
  • Follows appropriate charting and documentation requirements and agency regulations in a timely manner.
  • Provides input and feedback on potential related projects/programs, as appropriate.
  • Supports agency-wide Quality Improvement efforts.

Community Support and Advocacy:

  • Build trust and rapport with community members.
  • Serve as a role model and coach to inspire, engage and advocate for community members.
  • Provide psychoeducation regarding mental health, substance, child development, and parenting skills.
  • Provide culturally responsive support, including aiding individuals to understand and move through unfamiliar and complicated systems.
  • Support community members with completing applications for social services programs to access resources to meet their basic needs. 
  • Utilize motivational interviewing and other evidence based interventions to assist community members.
  • Collaborate with the team and other providers serving community members to ensure that all needs are being met.

Program Administrative Support:

  • Participate in team meetings, training sessions, and other meetings as required.
  • Provide appropriate documentation of services provided to clients.
  • Support with data entry and provide help with reporting.

Other 

  • Network with schools, districts and other community partners/staff to support objectives of the assigned program.
  • Attend trainings and participate in events relevant to key responsibilities
  • Track and regularly report on key metrics for responsible function areas
  • Collaborates on special projects related to agency objectives as appropriate 

Qualifications

QUALIFICATIONS & EXPERIENCE:

  • High School Diploma or equivalent preferred.
  • Lived Experience with mental health challenges, homelessness, and/or substance use recovery preferred (self or family member).
  • Ability to meet attendance requirements of the position and be timely in meeting all requirements for work performance. 
  • Bilingual in Spanish/English strongly preferred.
  • Ability to work in an independent environment.
  • Ability to learn web-based platforms (e.g., Salesforce) with appropriate training provided.
  • Non-Profit and Community Based Organization experience highly desirable.
  • Demonstrates and models excellent “customer service” orientation, especially in the ability to interact with people at all levels within and outside the agency.
  • Thinks independently and can make informed decisions when needed. 
  • Exercises extreme discretion with the ability to hold, balance and transmit public and confidential information needed by various parties. 
  • Proficient using MS Office products, particularly Word, Excel, Outlook and PowerPoint.
  • Acquires or maintains proficiency in using Google Drive products, including Docs, Sheets, and Forms.
  • Valid CA Driver’s license
    • Auto insurance:  Personal vehicle may be used for business related purposes; it is recommended you inform your auto insurance carrier.
