Essential Functions

  • Essential Duties 
    • Managing all aspects of the patient intake process including communicating directly with patients and families.
    • Entering data.
    • Establishing and maintaining positive relationships with customers and referral sources.
    • Responding to customer requests and concerns.
    • Managing the insurance verification and authorization process.
    • Ensuring compliance with all state and federal referral/intake regulatory requirements.
    • Utilizing electronic medical records to access and input patient information.
    • Sharing essential patient information with all members to coordinate care.
    • Coordinating with other departments for insurance verification and payer authorization as needed.
    • Communicating and following up with Business Development regarding pending admissions as needed or as directed by management team.
    • Assisting in orientation of new staff.

May perform other duties as required.

 

Qualifications

Minimum Qualifications

  • For non-clinical, a High School diploma or GED is required.
  • For clinical, a Diploma or associate degree required – Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse.  Current, active, and unrestricted license to that state the employee is assigned to work.  Current CPR required.  Minimum of two years of related experience.  Experience in direct care of patients with life limiting illnesses required.  Related experience may include hospice and palliative care, family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical. 
  • Computer proficiency is required.
