INTRODUCTION:
Day Habilitation Nurse/Registered Nurse- Supervisor Salary/Exempt
The Health Care Supervisor (HCS) is responsible for overseeing the indirect and direct nursing care provided to members overseeing Dayhab services from Amego Inc. The HCS will supervise or provide direct care and training in relevant areas and will coordinate medical services as needed with program members PCP (or medical service provider)/guardian/ residential group home provider. The RCS will oversee all heath care services provided at the program. The HCS will complete and supervise completion of nursing assessments. The HCS will participate in all interdisciplinary teams. The HCS will obtain reports and approval of medical are plans from PCPs. The HCS will ensure that nurse progress notes are recorded quarterly, or more often if the members condition requires more frequent monitoring. The HCS will advise the program director and other Dayhab Managers/staff of any medical problems that may hinder a member's participation in Dayhab or a specific activity and supervise any other nursing staff (LPN).
Primary Job Responsibilities Include:
- Oversee health care services for all program recipients. Coordinate medical services with each recipient's primary physician when needed.
- Member of inter-disciplinary evaluation team. Participates in ongoing interdisciplinary evaluations and review of each recipient's needs and status.
- Assist Program Director in securing necessary referral documentation and program authorizations for each recipient and in maintaining currency of records on an ongoing basis.

Maintain medical records, health plans, medical recommendations/authorizations and medication information for each program recipient as it pertains to the day habilitation service plans. Obtain reports and approval of treatment plans from primary physicians; and ensure that records/documentation meets regulatory requirements. Meet or exceed regulatory requirements for CARF, MASS Health and Day Habilitation programming.

Maintains notes, reviews and nursing assessments as required by regulatory standards.

Provide training and education to staff and individuals in relevant health care areas, including Safety, Universal Precautions, and others as requested by the Program Director or DHS.

Provide supervision to program staff, LPN (s), and function as therapy liaison person. Interpret medical information for staff and advise program staff of medical issues that may impact on a recipient's program participation.

Assist Program Director with training, supervising and evaluating assigned staff in accordance with agency policy and procedure. Reinforce a team approach, set clear expectations, provide feedback, communication and support to staff and individuals.

Participate in recipients' day habilitation service planning and other team meetings, as requested. Effectively communicate with state agency personnel, other service professionals, family members, guardians, etc.

Salary Information: RN Salary Range: $83,000-$89,440
LPN Range: $31-$33/hr
