Job Summary

Case Management Coordinator – Inpatient facilitates the transition of patients from inpatient facilities to the appropriate level of care by coordinating discharge planning, arranging follow-up, and addressing barriers to care. This role plays a critical function in reducing avoidable readmissions and supporting Sanitas’ value-based care strategy by ensuring smooth, patient-centered care transitions and strong alignment with providers and payor partners.

Essential Job Functions

Initiate discharge planning upon notification of patient admission.

Engage patients at inpatient or post-acute facilities and assess discharge readiness.

Collaborate with Hospitalists, discharge planners, and payer care teams to develop discharge plans.

Coordinate timely follow-up appointments with PCPs and specialists per discharge instructions.

Educate patients and families about care instructions, resources, and home-based options.

Identify and address barriers to safe discharge, including transportation and social needs.

Document interactions, plans, and care coordination activities in the EMR.

Monitor and report patient outcomes and care effectiveness to interdisciplinary teams.

Facilitate communication between providers, facilities, and community-based organizations.

Contribute to process improvement initiatives related to care transitions.

Supervisory Responsibilities

This position has no supervisory responsibilities

Required Education

High school graduate or equivalent.

Required Experience

3 - 5 years of experience in clinical, case management, or community health settings.

Experience in inpatient discharge planning or care coordination.

Qualifications

Required Licenses and Certifications

Medical Assistant License (required)

Required Knowledge, Skills, and Abilities

Strong communication, counseling, and documentation skills.

Proficiency in EMR platforms, Microsoft Office, and clinical tools.

Knowledge of medical terminology and post-acute care services.

Understanding of healthcare navigation and social determinants of health.

Ability to work collaboratively across teams and independently.

 

 

Preferred Qualifications

Familiarity with managed care and value-based payment models.

Local knowledge of community health and social service networks

Physical Demands

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.

Inside: The employee is subject to environmental conditions, protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout to be heard above ambient noise level.
