Summary of Position: The purpose of this position is to provide leadership to the clinical team in planning a patient’s discharge. The case manager is the member of the clinical team who is responsible for identifying the needs of the patient as they prepare for discharge, and is responsible for identifying resources in the community that may be used to meet those needs. Case managers assist the patient in breaking the cycle of homelessness by connecting them to any available housing options for which they may qualify. This is a safety sensitive position.  Â
Essential Duties:Â

Works as part of the clinical team with the primary responsibility of discharge planningÂ

Attends daily team huddles when in clinic, clinic staff meetings, and team meetingsÂ

Maintains appropriate documentation in the medical record so that the case management plan for each patient is easily accessibleÂ

Works with each patient on a comprehensive plan for discharge to permanent housing optionsÂ

Fosters empowerment and recovery by connecting the patient with community agencies to obtain services such as vocational rehabilitation, mental health and substance abuse services, job readiness and parenting skillsÂ

Accompanies patients on site visits with the goal of clarifying the discharge planÂ

Counsels patients on entitlements for which they may qualify and facilitates applicationsÂ

Keeps the clinical team updated on progress of each patient towards dischargeÂ
