Essential Duties and Qualifications

The Social Worker Assistant is responsible for planning, organizing, and implementing the program of social service welfare for the ultimate care of each resident in the facility under the supervision of the Social Worker.

 

Essential Duties & Responsibilities:
•    Schedule care plan meetings
•    Provide follow up contact to residents who have discharged from the facility. 
•    Performs general office duties including scanning and filing as needed
•    Performs other duties as assigned.

 

Qualifications and Requirements:
•    Must be able to perform the above stated essential duties and responsibilities. 
•    Must be able to lift, push, pull and move up to 25 pounds.

•    Oral and written communication skills 

 

Other Specifications:
•    Familiarity with medical and legal terminology.
•    Risk of exposure to bloodborne pathogens and hazardous materials.

•    Promote and demonstrate positive customer service towards co-workers, residents, families and visitors.
