POSITION OVERVIEW

As a Licensed Practical Nurse in Foster Care, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.

Responsibilities:

  • Communicate with medical director/providers regarding observation in client’s health/behavior.
  • Provide client and family education regarding medications and treatments.
  • Collaborate with our staff regarding concerns and behaviors.
  • Participate in Interdisciplinary Medication Review meetings.
  • Nursing documentation using EHR.
  • Update discharge planning needs.
  • See foster children in foster homes as needed.
  • Other duties as assigned within scope of practice and based on agency needs.

Requirements and Education:

  • LPN Licensed in New York State.
  • 1 to 2 years of experience.
  • Experience working with youth and adolescent population desired.
  • Must pass physical.
  • Must have a valid NYS drivers license and reliable transportation.
