Clinical Case Manager - Prevention

Northern Rivers Family Services

As a Clinical Case Manager for the Prevention program,  you will provide both Parent Education groups and Supervised Visitation to families referred by the Rensselaer County Department of Social Services.

You will provide classroom setting or virtual groups to caregivers with an evidence-based parenting curriculum (Active Parenting and Incredible Years), and home-based parent education and support. You will also provide supervised visitation with families within the Rensselaer County DSS office, our agency office, other community settings, and caregivers' homes. You will report to the Program manager and work a flexible, hybrid schedule including one-two early evenings per week and community travel.

Responsibilities:

  • Facilitate approx. 8 parenting groups, either in- person or virtual, per year
  • Supervise visits for up to 8 families based on each family's visitation schedule
  • Create service plans based on each family's needs
  • Complete paperwork within agency, county, and state guidelines
  • Collaborate with service providers and collaterals involved with the family, and attend service plan meetings
  • Testify in court when required

Requirements and Education:

  • Bachelor's degree in social work, sociology, criminal justice, psychology, human services or related field required
  • 1+ years working with at-risk youth and families, including home and community-based experience required
