Clinical Case Manager, Positive Youth Program

As a Clinical Case Manager in the Positive Youth Progression program, you will maintain a caseload of eight to ten youth and meet with each weekly. Positive Youth Progression provides six to nine months of home and community-based prevention services to youth who are at risk of being removed from their homes. The goals of the program are to prevent out-of-home placement for troubled youth and to avoid the juvenile justice system. You will report to the Program Coordinator and you will work with a partner (Clinician) in a combination of on-site and remote with travel to client's homes is required.

Responsibilities:

  • Complete assessments to identify strengths and needs of youth, caregiver and family
  • Develop treatment plans with the clinician for the youth and families to help identify and meet their goals
  • Assist caregivers in the development of effective parenting skills
  • Work with youth in the area of skill building (anger management, stress management and problem solving, social skills and emotional regulation) and independent living skills
  • A trauma-informed approach to service delivery and working with the clinician to conduct family sessions
  • Communicate with the youth’s family to monitor progress and support the family’s effort to keep the youth safe
  • Coordinate services with service providers and collaterals involved with the youth and their family
  • Complete paperwork within agency, county, and state guidelines.
  • You will work Monday through Friday with a flexible schedule that includes evenings and a rotating on-call schedule.

Requirements and Education:

  • Bachelor's Degree in Social Work, Sociology, Criminal Justice, Psychology, Human Services, or a related field
  • Experience working with at-risk youth and their families preferred
  • home and community-based experience is a plus
