We are looking for a highly organized and detail-oriented Home Health Intake and Scheduler to join our team and ensure the seamless scheduling of patient and clinicians visits and services.
Responsibilities:

Coordinate and schedule clinicians visits, assessments, and follow-up visits for home health services.

Communicate effectively with patients, families, and healthcare providers to confirm appointments and address any scheduling changes.Â

Work closely with clinical staff to ensure timely and efficient scheduling that meets patients needs and preferences.Â

Assist with intake processes and verify patient information and eligibility for home health services.

Maintain accurate and up-to-date records of patients in our EMR system

Ensure compliance with healthcare regulations and agency policies

Provide exceptional customer service to patients, their families, referral sources, and addressing any concerns or inquiries.

Participate in the office on-call rotation.

All other duties as assigned.
Requirements:
- Minimum of one (1) year of experience in health care (hospice, skilled nursing, MD office, hospital)
- Ability to work alongside a team of professional and administrative personnel
- Autonomous, organized, assertive, flexible, and cooperative in performing job responsibilities in a fast-paced work environment.
Job Type: Full-time
Pay: $21.00 - $23.00 per hour
