CLASSIFICATION: HVRP Case Manager (Salt Lake, UT and Phoenix, AZ)

DEPARTMENT: AMVETS NSF HVRP Department

EMPLOYEE STATUS: FLSA Exempt (Full-Time) Grant-Funded

ANNUAL SALARY: $48,000 per Annum

 

Organizational Relationships: 

AMVETS is seeking a motivated and compassionate Case Manager to join our Department of Labor-funded Homeless Veterans’ Reintegration Program (HVRP). You will be responsible for providing trauma-informed, employment-focused case management services to homeless veterans, at-risk veterans, and veterans with families. You will guide participants from crisis to career through a structured process of assessment, planning, employment preparation, and wraparound coordination. This position is under the direct supervision of the HVRP Team Lead Case Manager. This position is funded for a 12-month period, contingent upon continued grant funding.

 

Duties and Responsibilities:

  • Conduct intake and eligibility screening for HVRP participants based on program criteria
  • Complete needs assessments and co-develop Individual Employment Plans (IEPs) tailored to each veteran's employment goals
  • Assist with resume writing, interview prep, digital literacy, and connection to vocational training
  • Collaborate with SSVF, VA, HUD-VASH, American Job Centers, and local housing providers to deliver holistic services
  • Connect veterans with supportive services (e.g., childcare, transportation, mental health, addiction recovery, legal aid)
  • Track participants progress from enrollment through job placement and retention (up to 270 days)
  • Maintain accurate and timely documentation in HMIS and VETS Grantee Reporting System (VGRS)
  • Participate in weekly case reviews and collaborate with the employment team to drive job placement outcomes

 

Qualifications

Knowledge and Skills Required: 

  • U.S. Military experience preferred. High school diploma or GED preferred; relevant experience will be highly considered.
  • Minimum 2 years’ experience in case management, preferably with homeless or veteran populations
  • Familiarity with workforce development programs, VA benefits, and veteran-specific services
  • Strong written and verbal communication skills
  • Proficient with data entry and client tracking systems (e.g., HMIS)
  • Valid driver's license and reliable transportation for outreach and meetings

 

Physical and Environmental Demands:

This position requires an individual who can maintain a professional attitude while working on multiple assignments. While in the office, sitting for extended periods of time at the computer most of the day, occasionally standing, walking, lifting, needed to carry out everyday activities.

Must be available to drive and travel locally and attend evening and occasional weekend events as necessary.  Overnight travel may be required up to 25% of the time.
