Position Summary
The LVN coordinates and administers medical services to patients, including assessing, screening, educating and treating patient conditions.
Â
The LVN will also provide patient and client-centered medical case management services for individuals living with HIV/AIDS seeking or receiving services at WellSpace Health. The primary responsibility of this role is to ensure that clients have access to appropriate medical and psychosocial services as well as coordinated care to enhance overall health and well-being. The position emphasizes timely access to healthcare, continuity of care, and ongoing assessment of patient and client needs.

Required Qualifications
- Current license as Licensed Vocational Nurse with the State of California required.
- Current First Aid and CPR Certification or willingness to obtain within 90 days of employment.
- Knowledge of community resources including Medicare, Medi-Cal, Supplemental Security Income (SSI) and the county’s General Assistance Program.
- Knowledge of medications, usages, interactions and effects.
- Ability to observe, assess, and record symptoms, reactions, and progress.
- Ability to react calmly and effectively in emergency situations.
- Ability to educate patients and/or families and to provide instruction on proper care and treatment.
- Ability to maintain detailed records.
- Ability to establish and maintain positive, professional relationships.
- Proficient computer skills, including experience with Epic EHR preferred.
- Excellent verbal and written communication skills.
- Adherence to infection control procedures, including but not limited to standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking, and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Knowledge of HIV/AIDS, Hepatitis C, and related public health issues.
- Ability to conduct client assessments, develop care plans, and perform service coordination.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and in a team environment.
- Must be willing to successfully complete the credentialing process.

Essential Responsibilities
Â
- Act as on-site medical professional providing direct patient care according to the policies and protocols.
- Perform LVN duties for patients. Prepare exam rooms, and intake patients by performing assessments, take medical histories, record vital signs, take temperatures, discharge patients.
- Engage with community members experiencing a mental health crisis, homelessness, disabilities, and substance use.
- Encourage patients’ general well-being through health promotion.
- Follow standardized policies and protocols for patient treatments and conditions.
- Keep accurate records for reporting to various funding and inspecting agencies.
- Maintain files for regulatory compliance and inspection.
- Inventory medications and supplies; rotate inventory regularly to minimize expirations.
- Review patient medical charts and prepare documentation.
- Record all patient interactions in the EHR system and ensure accuracy for patient follow-up and billing.Â
- Assist with daily operations, laundry, food distribution, cleaning, etc.
- Perform other required nursing duties for patients under the influence of drugs and alcohol or in a mental health crisis.
- Provide medical case management services for patients living with HIV/AIDS (PLWH/A) under the clinical supervision of the Infectious Disease Physician.
- Organize and maintain patient databases to track HIV/AIDS, Hepatitis C and other sexual health diseases care progress.
- Compile and analyze information from EHR investigations, prepare reports, presentations, and maintain detailed records of findings with supervision from Clinical Quality Manager of Public Health.
- Uphold patient-centered care model to link patients to services to ensure timely, coordinated access to medically appropriate levels of healthcare and supportive services, continuity of care, and ongoing assessment of the patient’s and other family members’ needs and personal support systems.
- Assess the needs of patients living with HIV/AIDS using the "SSC 01 Medical Case Management Service Standards for People Living with HIV/AIDS (PLWH/A)."
- Develop individualized care plans in collaboration with patients, facilitate informed decision-making, and address both medical and social service needs to ensure smooth coordination of care.
- Meet weekly with the Infectious Disease & Sexual Health care team to review and adjust care plans based on patient progress and changing needs.
- Facilitate monthly care meetings with the Infectious Disease and Sexual Health (IDSH) team to review continuity of care for PLWH/A and Hepatitis C patients to ensure coordinated services across different providers.
- Host quarterly care meetings to coordinate care among multiple referring and service providers which will ensure comprehensive and integrated treatment.
- Collaborate with Patient Services, Compliance, Clinical Quality and other departments to analyze and resolve medical or clinical concerns to maintain high standards of care and quality.
- Using standing orders, assist with completion of clinical tasks, coding, documentation etc. including medical records management.
- Perform other duties as assigned to maintain and advance WellSpace Health’s goal of delivering high quality comprehensive care.

Physical Demands and Work Environment
The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.  The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
