Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.

 

Program Description: Goddard Riverside's Resource Center program is located in Central Harlem. The Resource Center helps low-income individuals and family's access and receive entitlements, benefits, and resources. The Resource Center also offers an array of services including, but not limited to, benefits screening, financial counseling, legal consultation and referrals to support services in the community and city wide.

 

Position Summary: To deliver effective, strengths-based, and client-centered case management services to low-income families and individuals, with a primary focus on serving older adults. The Senior Case Manager in this position will provide administrative support, guide team members and help implement improvements to enhance service delivery.

 

Schedule: Monday-Friday 9:00am-5:00pm

 

Salary Range: $47,000-$52,000

 

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

1) Client service

  • Conduct benefit screenings primarily for older adults as well as low-income individuals and families
  • Assist clients as they apply for benefits from local, state and federal agencies
  • Interview clients to assess social service needs and make referrals, accordingly, including referrals to in-house, legal clinics, financial counseling and other resources as needed.
  • Serve as an advocate for and liaison between clients and community organizations and/or government agencies
  • Work with clients who are seeking new housing opportunities, helping them fill out housing applications and referring them to the appropriate agencies and organizations.
  • Help keep clients in their homes through assistance programs as well as eviction prevention measures
  • Participate in community outreach events to identify and communicate with prospective clients; facilitate presentations to explain services provided at The Resource Center
  • Represent the Resource Center at Community Board meetings, events hosted by our elected officials and local CBO's activities participate in program evaluation and improvement.
  • Maintain detailed case notes
  • Track and provide outcomes for services rendered
  • Assist clients visiting the office and remotely
  • Attend meetings with funders to assess program progress
  • Conduct outreach and hold workshops at Older Adult Centers, NORC etc.

2) Administrative Duties

  • Assist the Program Director with reporting on different funding streams
  • Participate in training new case management staff
  • Complete case record audits monthly for the Program
  • Support the Program Director in serving as a Liaison to other community services; attend meetings and develop new linkages in the absence of the Program Director.
  • Serve as senior staff member in Director’s absence
  • Track and maintain records for staff training, and schedule staff needed.
  • Participates in ongoing training and educational opportunities; provides educational resources to staff and volunteer advocacy
  • Develop ongoing referral relationships with partner organizations and related CBOs

3) Resource Center Team member

  • Attend staff meetings, community meetings, case conferences, training programs and supervision meetings; participate in program evaluation and improvement
  • Work as a member of an active team and respond with flexibility to a fluid and evolving program environment
  • Assist in maintaining program office space and assist clients who visit the office.
  • In collaboration with Resource Center Team, work on marketing efforts to successfully engage tenants.

 

Skills, Knowledge and Abilities

  • Some college, bachelor’s degree preferred
  • Bilingual English/Spanish a plus
  • 1-3 years’ experience working with older adults and low-income populations in general knowledge of government benefits
  • Ability to multi-task & work in a fast-paced environment
  • Strong communication skills

 

Computer Skills: To perform this job successfully, an individual should be:

  • Proficient in Microsoft Office programs, especially Outlook, Word and Excel
  • Able to enter, manage and analyze data in spreadsheets and pull reports
  • Able to use or learn electronic databases such as Single Stop

 

Physical Requirements

  • Occasional lifting of up to 30lbs
  • Frequent walking and standing
