Coastal Hospice invites you to join a team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions.  In your role as Home Hospice Team Leader, you will:

  • Ensure a consistent high quality hospice experience
  • Collaborate with other members of the Coastal Hospice team to develop policies for the hospice residence compliant with state, federal and Joint Commission standards and best practices
  • Be a member of the Leadership Team and assists in administrative efforts to support the mission of the organization
  • Lead, supervise and mentor a team operationally and administratively, facilitating superior compassionate, efficient and effective delivery of services
  • Confer with appropriate members of the IDG regarding referrals, changes in patient level of care, contemplated discharges, patient hospitalizations and any other significant patient changes in a timely manner
  • Recruit, interview, hire and train staff members in collaboration with discipline specific clinical experts
  • Conduct employee performance reviews timely and effectively
  • Identify learning needs of staff and provides educational resources to meet employee needs; Serves as a resource and mentor to staff
  • Provide care as needed to hospice patients within scope of practice
  • Assure documentation of IDG members is complete and timely in accordance with legal and regulatory standards
  • Collaborate with leaders within Coastal Hospice to assure program needs are met and resources are used effectively
  • Participate in the development and implementation of protocols, policies and procedures related to patient care and organizational goals
  • In collaboration with supervisor, participate in annual budget process
  • Ensure department and organizational QAPI goals are met
  • Be accountable for meeting patient care needs 24/7 via appropriate scheduling and delegation to members of the IDG
  • Share in responsibility for management/leadership 24/7 including administrator-on-call (AOC) rotation
  • Assure smooth team functioning by:
    • Scheduling appropriate staffing levels according to budgeted position control
    • Oversight of adequate utilization of patient care equipment and medical supplies
    • Maintaining effective professional relationships with hospitals, physicians and other health care providers
    • Assuring patient related documentation is complete and timely in accordance with legal and regulatory standards
    • Maintaining safe physical environment according to unit needs and regulatory standards
    • Maintaining a relationship with partner facility and supporting joint collaboration

Qualifications

  • Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
  • Prior management/supervisory experience in a health care environment
  • Previous hospice experience preferred
  • Certification in Hospice and Palliative Nursing or appropriate discipline as it relates to role or attainment within two (2) years
  • Proficiency utilizing electronic medical records and Microsoft Office Suite
  • Emotional maturity to cope with family emotional stress and tolerant of individual lifestyles across a variety of cultural and socioeconomic backgrounds
  • Working knowledge of Medicare Hospice Benefit, Medicaid, private insurance, hospice service reimbursement practices and other community programs and resources
  • Excellent interpersonal and customer service skills
  • Excellent organizational and time management skills
  • Effective verbal and written communication skills
