NURSE DISCHARGE PLANNER

Mountain View Hospital is looking for a Nurse Discharge Planner to join our team!

JOB SUMMARY:

Works in close collaboration with physician health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patients health problems. Prepares patient and family for safe discharge from acute care hospital setting.

DUTIES AND RESPONSIBILITIES:

1. 1. Coordinate the discharge plan and services with the patient, their families and disciplinary teams to ensure that a patient has an appropriate plan and services when ready to discharge.
   2. Work with patients while obtaining maximum benefits from medical care by performing the following duties.
   3. Reviews and validates physician orders are carried out, reports progress and unusual occurrences on patients to department supervisor.
   4. Ensures cost effective and appropriate health care services are provided to all patients regardless of patients funding.
   5. Collaborates with other departments in discharge planning.
   6. Assists in assessing the physical, functional, social, psychological, environmental and financial needs of patients.
   7. Assists with referrals to appropriate community resources and contributes to the assessment of patient’s support systems.
   8. Acts as a patient advocate by assisting in acquiring appropriate medical, nursing or other health care in the home or clinic setting as needed upon discharge.
   9. Maintains accurate patient records, ensuring patient confidentiality.
   10. Contributes and assists in the performance of case management activities, and contributes to the assessing, planning, implementing, coordinating, monitoring and evaluating of the needs of patients for services both during and after hospitalization.
   11. Assists the department head in the supervision of the utilization of resources to promote quality, cost-effective outcomes for the patient and the patient’s family.

Education/Certification:  Graduate from an accredited school of nursing with current license as an LPN or Registered Nurse in the state of Idaho, with preferred experience in Case Management. Strong knowledge of modern nursing principle techniques, and procedures for the care of patients, with emphasis on growth and developmental needs of patients from birth to death. These skills may be obtained through formal educational programs, training or experience. A strong knowledge base is needed in medical terminology, anatomy and physiology, and concepts of disease.

Experience:  3+ years of clinical nursing.

Equipment/Technology:  Basic computer knowledge.

Language/Communication: Effective written and oral skills. Ability to comprehend and follow detailed instructions. Ability to effectively present information and respond to questions from groups of managers, patients, customers, and the pressure during difficult circumstances. Speaks clearly and persuasively in positive or negative situations: listens and gets clarification. Ability to be cheerful, trust worthy, loyal, organized, and demonstrate professional behavior at all times.

Mathematical Skills:  Business related math

Mental Capabilities:  Ability to organize, recognize, and correct Inefficiencies. Handle and prioritize multiple requests.

Interpersonal:  Can remain pleasant under extreme pressure and during difficult circumstances. Ability to be cheerful, trustworthy, loyal and organized.
