Mountain View Hospital is looking for a Population Health Manager to join our team!

JOB SUMMARY:       

Oversees daily operations of Population Health department and clinic efforts for all Valued Based Care arrangements including ACO, VCO, Narrow Network, PCMH, CCM and several quality based agreements with Commercial plans.

Qualifications

Education/Certification:  Bachelor’s degree in Health Care Administration or current licensure as a Registered/Practical nurse in the state of Idaho or relative certification or education.

Experience:  Minimum of three years medical office experience in family practice, one year supervisory or management experience and Value Based Care or Population Health experience strongly preferred. 

Equipment/Technology:  Must be able to use Microsoft Office, Word and Excel.

Language/Communication:  Must have the sensitivity, maturity, and ability to communicate clearly and concisely, both verbally and in writing.

Mathematical Skills:  General mathematics.

Performance:   Full time position requiring some weekends and holidays.
