TCI - SOCIAL WORK CASE MANAGER

Mountain View Hospital is looking for a Social Work Case Manager to join our team! 

JOB SUMMARY: This position provides support for the LCSW. And requires a passionate commitment of core values and behaviors. Responsibilities- Coordinates and assists with new patient set up and resource contact for LCSW. Functions as a Case Manager for the Social Worker. DUTIES AND RESPONSIBILITIES:

Title:  SOCIAL WORK CASE MANAGER

1. Helping the LCSW in assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plan.

2. Facilitates flow of information regarding patient’s course among care providers.

3. Keeps strict confidentiality with patients information and follows all HIPAA laws

4. Listening to clients’ concerns and providing resources or referrals as required.

5. Following up with patient’s progress periodically, collaborating with the LCSW and making adjustments as needed to improve outcomes.

6. Keeps a spreadsheet of patients who have been referred out for counseling.

7. Helps schedule patients to be seen with the LCSW for counseling.

8. Scans/Copies and uploads documents into EMR Aria and Cerner.

9. Helps medical staff provider resources to patients.

10. Maintains client contact list. And updates folders as needed.

Please note benefits are based on eligibility according to full-time, part-time or PRN status classification. 

Education/Certifications: Must be a high school graduate or an accredited secondary educational system. Preferred C-SWCM (certified social work case manager) must have a working knowledge of the Microsoft Office suite products (Word, Excel, & Outlook).  Knowledge of basic Workers Compensation and medical terminology is essential.  Must have a current driver’s license.

Experience:  Knowledge of Medical practice, and Medical Terminology.
