JOB SUMMARY:Â This position provides support for the LCSW. And requires a passionate commitment of core values and behaviors. Responsibilities- Coordinates and assists with new patient set up and resource contact for LCSW. Functions as a Case Manager for the Social Worker. DUTIES AND RESPONSIBILITIES:
Title:Â SOCIAL WORK CASE MANAGER
- Helping the LCSW in assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plan.
- Facilitates flow of information regarding patient’s course among care providers.
- Keeps strict confidentiality with patients information and follows all HIPAA laws
- Listening to clients’ concerns and providing resources or referrals as required.
- Following up with patient’s progress periodically, collaborating with the LCSW and making adjustments as needed to improve outcomes.
- Keeps a spreadsheet of patients who have been referred out for counseling.
- Helps schedule patients to be seen with the LCSW for counseling.
- Scans/Copies and uploads documents into EMR Aria and Cerner.
- Helps medical staff provider resources to patients.
- Maintains client contact list. And updates folders as needed.
