BASIC FUNCTION: Directs Nursing Department in the Health Care Center, Assisted Living Center, In-Home Assistance and residential apartments to maintain quality standards of care, and advises medical staff, department heads and Administrator in matters related to nursing service

JOB REQUIREMENTS:
A. Education. Current license to practice as a registered nurse or vocational nurse in the State of Texas; Registered nurse preferred. Certification in CPR preferred.
B. Experience. Two (2) years clinical experience; long term care experience preferred.
C. General Requirements. Must be capable of executing all terms and conditions set forth in the Brazos Presbyterian Homes Personnel Policy Handbook and Policies and Procedure Manual, including but not limited to:
1. Works in a safety conscious manner which ensures that safe work practices are used in order not to pose a risk to others in the work place.
2. Interacts in a tactful, diplomatic, and humanistic manner with residents, families, visitors, and co-workers to provide a safe, efficient, caring and therapeutic environment which ensures the self-respect, personal dignity, rights and physical safety of each resident and center guest.
3. Maintains a dependable attendance record and adheres to standards of cleanliness, grooming, hygiene, and dress code
4. Complies with Brazos Presbyterian Homes/Bayou Manor policies and procedures, and local, state, and federal regulations.

6. Conducts self in professional manner.
D. Physical Demands. Lifting 50 pounds maximum and occasionally lifting and/or carrying objects weighing up to 25 pounds; reaching, handling, feeling, bending, stooping, and seeing. Extensive walking and standing.
E. Working Conditions. Inside in well-lighted, well-ventilated area.
F. Principal Contacts. Nursing staff, residents, and families.
JOB SPECIFIC RESPONSIBILITIES:
A. Delivery of Resident Care
1. Participates in assessing, planning, implementing, and evaluating resident care.
2. Completes initial and ongoing nursing assessments needed to plan care interventions.
3. Makes frequent rounds to ensure care delivery meets quality assurance standards and restorative protocols.
4. Uses appropriate infection control techniques to prevent spread of infection.
5. Administers prescribed medications and treatments in accordance with approved nursing protocols and within the scope of practice.
6. Monitors vital signs, hydration, and nutritional status to detect deviations from normal limits.
7. Can perform duties as outlined for nursing assistants; able to teach and correct nursing assistants’ daily duties as needed.
8. Assembles and utilizes resident care equipment and ancillary supplies.
9. Obtains specimens for laboratory exams, within scope if practice.
10. Takes, transcribes, and implements physician’s orders.
11. Requisitions and arranges for diagnostic and therapeutic services as ordered by resident’s physicians.
12. Orders prescribed medications and requisitions supplies and equipment from appropriate sources.
13. Assesses resident’s responses to treatments/medications, response or lack of progress toward care plan or restorative goal.
14 . Perform admission, give vaccination to residents.
15. Administer parenteral, intravenous, and intramuscular and subcutaneous injections; take and record vital signs and monitor vital signs for deviations from normal limits.
16. Order all new medications and refills; give initial dose of all new medications and document it timely and accurately.
17. Perform admissions, skin assessment.
18. Perform discharges, and do documentation, prepare medications for discharge.
B. Supervision of Staff
1. Participates in the assessment, planning and implementation of nursing staff efforts in providing quality resident care. Able to teach and correct techniques to maintain quality of resident care.
2. Participates in planning and implementing the coordination and involvement of nursing staff and other departments or services to teach family to participate in or manage resident care.
3. Coordinates activities between department and other services.
4. Monitors performance of nursing personnel against established standards and provides guidance to improve care delivery.
5. Assists with orientation, instruction, and supervision of other nursing personnel.
6. Motivates employee productivity; reinforces lines of communication and supervises employees.
7. Promotes employee productivity; reinforces lines of communication and supervises employees.
C. Coordination of Services
1. Assists physicians during rounds and examinations.
2. Coordinates and implements the physician services for each resident.
3. Coordinates and evaluates the supportive interventions provided by nursing and other services in a holistic approach to resident care.
4. Communicates well with nurse practitioner and other departments.
D. Reporting of Information
1. Monitors resident’s progress, lack of progress, or changes in condition describing findings in the medical record, to immediate supervisor, nurse practitioner and physician.
2. Initiates and coordinates appropriate communication concerning residents to and from family and significant other.
3. Communicates evaluation of findings and proposes adjustments in the interventions to all care team members.
4. Participates in planning and conducting training for other nursing personnel.
5. Observes residents, records significant changes in condition, abnormal lab values, responses to treatments/medications, and significant incidents and notifies nursing supervisor, physician, and family of change, when indicated.
6. Gives and receives comprehensive report of resident’s condition.
7. Coordinates telephone inquiries.
8. Reviews, according to standards, the resident care plans with resident, supervisor, resident’s physician, certified nursing assistants, and the resident’s family, when available.
9. Maintains unit records and performs charting duties.
10. Maintains narcotic records; completes required forms.
11. Reports immediately to Director of Nursing or Assistant Director of Nursing any verbal or physical abuse heard, seen or suspected for immediate investigation.
BRAZOS PRESBYTERIAN HOMES/BAYOU MANOR RESPONSIBILITIES:
1. Participates in departmental quality assurance activities.
2. Reports observations concerning structural, equipment and furniture defects and malfunctions to appropriate personnel.
3. Attends and participates in scheduled in-service programs and monthly staff meetings with Director of Nursing.
4. Maintains confidentiality of necessary information.
5. Utilizes supplies and equipment properly and without waste.
6. Thinks and acts calmly and logically to meet unusual occurrences of the job.
7. Performs any miscellaneous work assignments as may be required.
8. Attends monthly staff meetings with Director of Nursing.
9. Responsible for keeping current license and required CEU’s.
Other Tasks:
- Respond to questions from facility personnel accurately and timely.
- Accurately prepare and proofread memos, letters, spreadsheets and other documents and correspondence as required. Complete accurate and timely documentation. Review and accurately process mail, faxes, emails, and other correspondence in a timely manner.
- Answer phones, take messages, and respond to requests timely.
- Maintain compliance with all BPHI policies, procedures and requirements. Maintain compliance with all federal and state laws. Maintain compliance with all agency regulatory guidelines and requirements.
- Attend and participate in trainings and staff meetings as required. Conduct staff meetings as required.
- Develop emerging leadership through professional growth and development.
- Responsible for supporting and representing BPHI at special events, activities and other assigned functions.
- Work successfully as a part of a team and responsible for developing then maintaining strong working relationships with co-workers.
