Position Summary:
The Fortune Academy is designed to assist residents in making the transition from criminal justice history to positive, legitimate and productive lives. The Case Manager is responsible for the day-to-day activities of his/her assigned caseload. These activities include, but are not limited to: screening, assessing, counseling and linking homeless, formerly incarcerated residents with histories of mental illness, substance use and/or HIV/AIDS to needed permanent housing. Under the direct supervision of the Manager of Residential Service Case Manager will be responsible for the following:

JOB TASKS:

Facilitate individual and group counseling sessions;

Aide clients in accessing community resources, including public assistance, housing, day care, etc;

Maintain accurate records in a timely and orderly fashion;

Maintain liaisons with relevant city, state and private not-for-profit agencies;

Write court reports;

Work with clients in developing treatment plans;

Perform urinalysis; and
-
Other tasks and responsibilities as the job may require.
