Job Summary

The full-time Mobile Addiction & Recovery Counselor position is responsible for providing community-based substance use disorder services including outreach, screening, assessment and treatment planning of participants who are receiving services to address substance use disorders. This position will work closely with the participant’s peer mentor, case manager, and community partners as needed to provide care coordination services. This position collaborates with internal and external community partners and a multi-disciplinary team of BTC providers to ensure the best possible outcomes and care for participants.   

Essential Job Duties and Responsibilities

  • Serves a diverse population effectively, through trauma informed, culturally responsive and evidenced-based approaches to care.
  • Conducts substance use disorder screening & assessments and makes diagnoses using DSM-V-TR criteria and ASAM PPC
  • Develops individualized service (treatment) plans for BTC participants in consultation with the participant, other BTC providers, the participant’s family (whenever appropriate), and representatives of BTC partner organizations
  • Completes and maintains accurate and same-day treatment documentation including individual service notes; services and supports plans; court reports or reports to probation/parole; assessments; and closure summaries that meet OAR/Medicaid standards
  • Organizes and manages schedule with the goal that a minimum 50% of hours worked are spent providing direct face-to-face services with BTC participants, in the location of their choice in the community.
  • Provision of culturally responsive individual, family and group counseling services congruent with BTC clinical orientation and care models. Maintains fidelity to evidence-based treatment models and practices adopted by BTC
  • Builds relationships in communities that foster trust and connection, as to best support underserved populations and community members 
  • Safely engages with individuals who may be under the influence of substances, are currently unhoused, recently released from the hospital, and/or any other high risks needs community members may experience using Assertive Community Treatment and harm reduction principles of care
  • Provides crisis screening, assessment and intervention services to clients to assist in minimizing periods of crisis, severe dysfunction and acute care utilization
  • Engages in care coordination and case management with other service providers internally and externally, and assists clients in developing and accessing natural community supports and community resources
  • Drives and operates the Mobile Behavioral Health Team vehicles safely in the community
  • Attends and actively participates in individual and group supervision, weekly MDT & staff meetings, and other agency meetings as assigned
  • Completes agency assigned training requirements on a timely basis and maintains licensing requirements necessary for the position and agency requirements.  Attends seminars, training, and other educational opportunities in order to develop skills and abilities.
  • Abides by agency policies and procedures and all relevant state and federal regulations and professional ethical standards, including mandatory abuse reporting, and HIPPA requirements.
  • Performs other duties as assigned.

Experience, Knowledge, Skills and Abilities

The following are the minimum levels required to successfully perform the Essential Job Duties and Responsibilities.

 

  • CADCI certification with MHACBO licensing board, required
  • State ORCHARDS BCU background check clearance, required
  • Experience with substance use counseling, preferred
  • Well-developed clinical documentation skills and a strong commitment to quality assurance, and clinical compliance, required
  • Experience working with peer mentors in a behavioral health setting, preferred
  • Experience with community outreach in underserved communities, preferred
  • Experience working with Assertive Community Treatment and Harm Reduction models, preferred
  • Experience serving vulnerable, high-risk populations using evidence-based practices.
  • Skilled in engaging and working with individuals experiencing acute behavioral health symptoms
  • Knowledge of comorbid substance abuse and mental illness diagnosis and treatment using DSM-5-TR and ASMA PPC
  • Demonstrated success at working effectively in a diverse, collaborative team environment, with skills promoting inclusion and cultural responsiveness
  • Experience collaborating with community partners to build partnerships and improve participant outcomes
  • Ability to consistently maintain high ethical and professional standards and communication with participants, staff, and community partners
  • Computer literate; functional ability with Word, Outlook, and the ability to maintain an Electronic Health Record (EHR) that assures compliance w/ agency, state and federal regulations.
  • Understanding of outcomes-based care and implementation of tools to assess outcomes

Working Conditions

Environment and Physical Requirements: The Mobile Addiction & Recovery Counselor works in a hybrid community and office-based environment, and interacts constantly with staff, visitors, program participants, government agencies, and others in a broad range of settings. The position stands, walks, bends, lifts, and moves intermittently during working hours. The incumbent must be able to lift, push, pull and move files and equipment in excess of 15 pounds.

Position Type/Expected Hours of Work: This is a full-time position. Days and hours of work are typically forty hours per week with flexibility in scheduling, based on business need. The Addiction & Recovery Counselor position travels frequently in the community.
