About the Role:
Join our HOTT program as a Case Manager and make a meaningful difference in the lives of LGBTQ+ and marginalized youth. You'll provide essential case management and supportive services to young people ages 13-24.
Key Responsibilities:
As a HOTT Case Manager, your responsibilities will include:

Register new patients and enter their data into our electronic health records.

Assess patients' eligibility for our low-cost or no-cost services.

Assist patients in enrolling in government-funded health insurance programs.

Coordinate referrals to other healthcare providers and community resources.

Provide education and support on family planning and various medical issues.

Work closely with primary care providers, nurses, behavioral health workers, and outreach staff to ensure comprehensive care for patients.
Work Schedule/Salary:
This is a full-time, 5-day-per-week position including some evening hours. You must be available for early morning, evening, and/or weekend shifts.
