About the Role:

Callen-Lorde is seeking skilled and patient-centered Registered Nurses (RNs) to join our Digital Care & Clinical Support team. In this innovative role, you will provide high-quality nursing care through electronic communication, ensuring patients receive timely support, education, and follow-up even when they’re not physically in the clinic.

Instead of primarily providing direct in-person care, this position focuses on managing digital workflows such as patient messages, medication inquiries, lab result communication, and care coordination across the network. You will work closely with providers and care teams to streamline communication, address patient needs, and support continuity of care.

Key Responsibilities:  

As an RN, your responsibilities will include:

  • Monitor and respond to patient messages in the EHR related to symptoms, medications, and care needs.

  • Use clinical judgment to triage concerns and escalate urgent issues to providers or care teams.

  • Manage orders (labs, refills, referrals) per protocol and provider guidance.

  • Provide patient education and support through secure messaging or phone.

  • Coordinate follow-up appointments, referrals, and diagnostic testing.

  • Accurately document all patient interactions and nursing assessments in the EHR.

  • Perform basic on-site clinical tasks at least one day per week to support providers and maintain skills.

  • Participate in workflow and quality improvement initiatives to enhance efficiency and patient experience.

 

Work Schedule:

RNs in this role typically work remotely, with at least one on-site day per week to maintain clinical skills, stay connected to onsite workflows, and strengthen collaboration with in-person teams.

Qualifications

  • New York State RN License         
  • Minimum 2-3 years of acute or ambulatory care experience is required. Experience in ER, telephone triage, public/community health care setting is preferred
  • Current CPR certification
  • Strong computer competency
