Description

We are seeking a Teen Home Finder to join our Foster Care program at HSVS’. The Teen Home Finder will be responsible for ensuring that our program youths in foster care have a safe, healthy and nurturing foster home.

Primary Responsibilities and Essential Functions of Position:

  • The Teen Home Finder will ensure that all agencies, city, state, and federal regulations and requirements are strictly followed and accurately documented.
  • Home Finders ensure that foster families feel supported and valued while providing loving care for every child in their home. Home Finders provide training and guidance to foster families, to help them to raise happy, healthy and secure children.
  • The Teen Home Finder will specialize in teen foster families.
  • They will recruit foster parents interested in caring for teens through both community and targeted recruitment and ultimately supervise those homes.
  • The Teen Home Finder shall facilitate the step down of youth from the HSVS Healing Homes, including but not limited to meeting regularly with Healing Home staff, identifying potential foster families for youth ready to step down, scheduling meet and greets, facilitating pilot period meetings and updates, and ultimately placement. Teen Home Finder will oversee and support teen homes, including conducting recertification annually.

Home Certification:

  • Complete detailed, accurate and timely written assessments on all foster parents and foster homes assigned to them, including annual reauthorization narratives, home studies for newly trained and kinship homes, and addenda for all homes, as needed.
  • Visit foster homes to ensure safety, regulatory compliance, and provide foster parent support.
  • Ensure that all foster homes on their workload have foster home case record files that are complete, accurate, tidy and audit-ready at all times.
  • Being the responsive and proactive point of contact for all the foster families on their individual workload,
  • Assist and support foster parents in meeting challenges and achieving success, through participation in conferences, administrative meetings, referrals for additional services, or other interventions, as necessary.
  • Receive training and certification allowing home finder to deliver required foster parent training and subsequently deliver said training as requested.
  • Deliver training and structured foster parent support.
  • Enter data into required electronic systems, including Connections and Binti.

Home Recruitment:

  • Conduct community based and targeted foster family recruitment for teen youth in care.
  • Facilitate the step down of youth from the HSVS Qualified Residential Treatment Program (QRTP) Healing Homes, including but not limited to meeting regularly with Healing Home staff, identifying potential foster families for youth ready to step down, scheduling meet and greets, facilitating pilot period meetings and updates, and ultimately placement.
  • Meet and collaborate regularly with Healing Home staff, WWK recruiters, Intake staff, Foster Family Recruiters, and Intensive Foster Parent Support Specialist.

Qualifications

  • Bachelor's Degree required
  • Experience in foster care preferred
  • Strong organizational and multitasking skills, with the ability to prioritize tasks effectively.
  • Excellent written and verbal communication skills.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) 
  • Ability to work independently while maintaining collaborative relationships with team members.
  • Strong attention to detail and problem-solving abilities.
