SUMMARY:

In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Department Director or Coordinator, assist in the provision of quality care for people served by performing the following duties.

 

 

 

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).

 

  • Case Management
  • Complete a weekly schedule for 1:1 session with clients
  • Conduct hourly rounds of the facility   
  • Assist client with completing housing applications for those that qualify.
  • Conduct self help groups daily
  • Meet with clients weekly or as needed
  • Attend DMHAS training along with Relias training
  • Complete DDAP at admission, six months review and at discharge
  • Maintain timely TCM notes, monitor client progress primarily regarding housing but may include finances, employment, nutrition, basic needs, physical health, mental health and substance abuse related issues.
  • Serve as an advocate for clients in obtaining necessary resources and services.
  • Maintain a current list of local landlords, realtors, and property management companies for potential apartment options.
  • Participate in weekly staff meetings
  • Monitor medication

 

  • Knowledge of best practices in providing services for individuals with mental health and substance use disorders such as clients centered care, motivational Intervention and trauma informed care.
  • Assumes all other appropriate duties as deemed necessary by the supervisor.

 

 

 

QUALIFICATIONS:

  • Ability to work with diverse populations. Ability to work independently or with a group. Assertively link individuals to housing and community resources. Ability to engage local landlords, explain subsidies process and advocate for landlord participation. Ability to coordinate multiple tasks simultaneously. Highly motivated, organized, creative with excellent interpersonal skills. Bilingual/Bicultural preferred.
  • Commitment to the mission of ending homelessness.
  • Knowledge of principles and techniques of social service case management services.
  • Knowledge of the physical, psychological, social and economic effects of homelessness, substance abuse, physical and emotional illness, crisis intervention. 

 

 

 

EDUCATION AND/OR EXPERIENCE:

 

  • Bachelor's degree in behavioral sciences and two (2) years’ experience in counseling preferred, or equivalent combination of education and experience.  Co-occurring competencies and or credentials are preferred.
  • Incumbents may be required to secure professional certification and/or license. 
  • Valid Connecticut driver's license is to be required.

 

 

 

PHYSICAL REQUIREMENT AND WORK ENVIRONMENT:

The physical requirements and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This position requires the manual dexterity sufficient to operate phones, computers, and other office equipment. The position requires the physical ability to kneel, bend and perform light lifting. This person must have the ability to write and speak clearly using the English language to convey information and be able to hear at normal speaking levels both in person and over the telephone. Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.

 

Qualifications

CORE COMPETENCIES

Bachelor's degree in the behavioral sciences and two (2) years experience in counseling preferred; or equivalent combination of education and experience.  Co-occurring competencies and or credentials are preferred.

 

  • Knowledge of principles and techniques of social service case management services.
  • Knowledge of the physical, psychological, social and economic effects of substance abuse, homelessness, physical and emotional illness, crisis intervention. 
  • Ability to gather, analyze and evaluate information pertinent to client care. 
  • Knowledge of community resources. 
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality. 
  • Ability to establish and maintain cooperative professional relationships. 
  • Excellent communication skills. 
  • Ability to project a professional image.

 

 

CERTIFICATES, LICENSES

Incumbents may be required to secure professional certification and/or license.  Valid Connecticut driver's license may be required.
