CHC-Care Manager

We at Wayne Memorial Community Health Centers are seeking two Care Mangers to join our team and support chronic care management as well as our Value Based Care Initiative Programs including transition of care management (TCM), following up on Emergency Room discharges, care gap closures, and completion of Annual Wellness Visits (AWVs). 

As a Care Manager, you will participate as a core member of the collaborating care team to enhance the overall physical and mental health of patients by providing comprehensive, individualized care, coordinating treatment plans and connecting patient/caregivers to essential resources. This will be accomplished through a myriad of functions including but not limited to:

• Identifying and prioritizing the patients’ needs and preferences, develop quantifiable goals and desired outcomes and promote the patient’s ability to self -manage to the greatest extent possible.
• Collaboration with and referring patients to a variety of resources to address social determinants of health, specialty medical, mental health, and/or substance abuse needs; tracking and managing patients’ referrals and follow up.

Job Requirements:

• Graduate of an accredited nursing program (LPN) with current licensure and/or a bachelor’s degree in social work or a related field.
• Demonstrates high degree of clinical and leadership competence.
• Minimum of three recent years in healthcare organization or social services field.

• Demonstrates interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.
• Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization.

• Demonstrates ability to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.

• Knowledge of chronic conditions, medical terminology, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education and ability to manage complex clinical issues utilizing assessment skills and protocols

• Possesses intermediate computer skills (operating systems, software, and applications) and demonstrates a willingness to learn more advanced skills
• Ability to work independently, while collaborating with other team members. Serves as a clinical resource person to staff and patients/caregivers.
• Willingness to establish and maintain effective working relationships with internal and external resources.
• Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner.
