- Schedule: 17Â hours (late afternoons)
- Location:Â 320 Carleton Ave in Central Islip
- Reports to:Â Program Director

What drives the core purpose of this job:
The Peer Specialist is responsible for providing peer support to families and assisting the Case Management Unit in all aspects of its operations in the Home Base (Intensive)Â Program.

What are the key responsibilities:
- Provide peer support to families in crisis struggling with problems such as substance abuse and mental illness. Peer support includes home visits, telephone reassurance, advocacy and encouragement for clients.
- Take clients on recreational outings and assist with facilitating the adolescent groups.
- Ensure clients receive required visits per month as per Director.
- Adhere to flex fund and petty cash procedures.
- Facilitate and develop monthly educational workshops for adolescents when assigned by Director.
- Maintain complete client records and have available for inspection by Supervision at any time.
- Provide crisis intervention when needed.
- Provide support to program staff and referral agencies.
- Maintain professionalism when interacting with families and all collateral contacts.
- Participate in team meetings, case conferences and training's.

What key knowledge, skills, and abilities are required to excel in this role:
- High School Diploma or GED required. Persons in recovery must have a minimum of three years sobriety as per funder requirements. Must have knowledge of treatment and mental health programs.
- Working knowledge of criminal justice issues, mental illness, substance abuse, and/or adolescent behavior preferred.
- Excellent communication skills, both oral and written; Ability to work cooperatively with criminal justice, treatment and mental health systems.
- Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications calendar, and email and/or database software necessary to perform work assignments.
- Must have access to automobile and possess a valid New York State driver’s license.
