What You’ll Do:

  • Evaluate and support residents and families with social, emotional, and psychosocial needs
  • Provide guidance in areas such as conflict mediation, crisis management, abuse and neglect prevention, advance directives, and community resource connections
  • Collaborate as part of an interdisciplinary team to promote engagement, understanding, and adaptation to care needs
  • Participate in care planning and assist with Individualized Service Plans (ISPs)
  • Maintain accurate, detailed documentation in electronic medical records
  • Support new admissions and transitions by partnering with the Director of Community Relations
  • Educate staff and families on psychosocial topics and resident well-being
  • Uphold Heritage’s mission, vision, and core values in every interaction

 

What Makes a Social Worker Successful at Heritage:

  • Ability to work both independently and collaboratively
  • Strong understanding of geriatric care and senior living settings
  • Knowledge of Wisconsin state regulations for CBRF and/or RCAC

 

Qualifications:

  • Bachelor’s degree in Social Work or related field required
  • Licensed Social Worker (LSW or equivalent) in the State of Wisconsin preferred
  • Experience in long-term care, assisted living, or senior services strongly preferred
