Description

We are currently seeking a full-time Case Manager/Skills Instructor float. This position will work 20 hours as a Case Manager and 20 hours as a skills instructor. The candidate will work to build and foster professional relationships with individuals served on her/his caseload and members of each individual’s interdisciplinary team to ensure ongoing coordination of services provided by Life Choices in accordance with Life Choices mission.

Monday – Friday, 8:00am – 4:00pm

ESSENTIAL DUTIES AND RESPONSIBILITIES
CASE MANAGER

  • Build and Foster professional relationships with individuals on her/his caseload, family members, and members of the DHSP team to maximize an individual’s human dignity, community integration and opportunities for self-direction.
  • Monitor follow-through and effectiveness of support plans with department staff and Interdisciplinary team in cooperation of the department manager. Provide guidance to department staff in competing required documentation.
  • Route all required documentations to members of the DHSP team such as DHSP sign off sheets, incident reports, etc. File all required paperwork in the Individual confidential file.
  • Actively provide input and coordinate with other departments and external referral sources to assure needed and desired services are being provided.
  • Plan for and participate in planning meetings (team meetings, DHSP, ISP) actively advocating for Individuals’ needs, and contributing to overall goals of the department and individuals supported.
  • Participate and assist with intake process and orientation of new individuals to Life Choices services.
  • Coordinate or complete assessments/evaluations in order to determine support levels and support planning. Conduct re-evaluations on an as needed basis.

SKILLS INSTRUCTOR

  • Provide personal care and assistance to individuals consistent with both their physical needs and defined support plans. Ability to adapt and make adjustments to techniques based on functional abilities and limitations.
  • Develop and implement community based activities that promote personal choice, community inclusion, dignity, and respect while enhancing personal skill development. Provide transportation as requested by Manager.
  • Actively participate in interventions, providing positive, knowledgeable support to staff and managers, ensuring crisis plans and behavior support plans are followed. Complete incident and accident reports in a manner which clearly identifies all components of the intervention and in accordance with regulatory expectations.

Qualifications

  • Bachelor’s Degree in Human Services and 1 year experience working with Developmental Disabilities
  • Experience working with families of individuals and professional service providers is preferred.
  • Working knowledge of OSHA, DPH, CARF, QUEST, DDS, EOEA (Medicaid) policies and procedures preferred.
  • Valid Driver license, acceptable driver record and reliable transportation.
  • Acceptable CORI report required.
