Description

The 412 Youth Zone offers comprehensive, welcoming and youth-oriented services and space for transition age youth with current or past connections to child welfare or who are experiencing homelessness.  The 412 Youth Zone helps youth to feel welcomed, safe and supported while offering the services and connections to help them face the challenges of adulthood, including education, employment, financial literacy, housing, health, mental health and substance abuse, personal relationships, parenthood and legal assistance. The 412 Youth Zone brings these offerings together in one location through a combination of:

  • Core services and supports
  • Specialized services offered on-site through sub-contracted agencies
  • Teaming with other DHS programs

The following objectives will be achieved:

  • Provision of high-quality programming
  • Assurance of a capacity sufficient to meet the needs of the target population
  • Excellence in youth engagement in supportive programming
  • Streamlined collaboration among agencies that serve the target population
  • Improved efficiencies in service delivery to the population
  • Assurance of client access to co-located services and flexible hours
  • Focus on youth outcomes, particularly in the following areas:
  • Graduating high school
  • Increasing youth enrollment in post-secondary education
  • Obtaining and retaining self-sustaining employment
  • Permanent and stable housing
  • Promoting health outcomes and discouraging teen pregnancy
  • Enhancing life skills that are critical to achieving self-sufficiency

 

ESSENTIAL RESPONSIBILITIES:

  1. Rapidly engage new referrals, gather information needed to determine eligibility, and promote the 412 Youth Zone to help youth understand the benefits of participation in the program.
  2. Enter new referrals and intakes into case management system.
  3. Work collaboratively with supervisors and managers to ensure new youth are assigned in a timely manner by completing paperwork and clearly communicating youth needs.
  4. Assess and build relationships with individuals and services inside and outside of the Youth Zone and assure that youth have access to needed services to accomplish the Youth Zone objectives above.
  5. Build strong consistent relationships with youth.
  6. Use the services and partners in the 412 Youth Zone to engage youth.
  7. After completing intake and determining eligibility, ensure a warm hand-off to the assigned youth coach.
  8. Complete comprehensive high-quality documentation as required within time expectations.
  9. Promote the philosophy and mission of Auberle in the provision of all responsibilities.
  10. Promote the Continuous Quality Improvement philosophy and maintain all Quality Improvement goals and objectives.
  11. Demonstrate an understanding of and respect for the cultural diversity of consumers, volunteers, staff, and other stakeholders throughout the provision of services.
  12. Assure compliance with Council on Accreditation (COA) Standards, Sanctuary Principles and Values, State Regulations, County Contracts, and Agency Policies and Procedures.
  13. Perform such other duties as the CEO, COO, or director may direct.

TYPICAL WORKING CONDITIONS:

  1. Requires working flexible hours, including evenings, weekends and holidays.
  2. Work is performed in an office environment, drop-in center, the community, and youth homes.
  3. Requires frequent travel to youth homes and throughout the community.
  4. Involves exposure to tobacco smoke, domestic animals, household pests, and other conditions common to domestic environments.
  5. Constant contact with staff and non-staff (e.g., client families, referents, staff from other agencies)

Qualifications

QUALIFICATIONS/REQUIREMENTS:

  • Commitment to and passionate about serving transition age youth
  • HS diploma with 2 years experience working with youth, Associate’s degree with 1 year of experience working with youth, Bachelor’s degree preferred, experience working with youth required (Special consideration will be given to individuals with previous DHS system involvement as a client.) Other combinations of combination of education and experience will be considered.
  • Valid Pennsylvania’ driver’s license, vehicle, registration and insurance
  • ACT 33/34 and FBI Clearances
  • Random drug testing may be conducted to maintain a safe and drug-free workplace.
