POSITION PURPOSE:
The Outpatient Behavioral Health Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care to individuals receiving mental health and substance use treatment services in an outpatient setting. The LPN supports the interdisciplinary care team by assisting with clinical procedures, coordinating care, monitoring patient progress, and promoting an environment focused on dignity, safety, and recovery.
QUALIFICATIONS:
- Graduate of accredited practical nursing program
- Current LPN license in State of Nebraska
- Basic Life Support (BLS)

Essential Job Duties and Responsibilities
1. Utilizes the nursing process, for the performance of patient care, to schedule, plan and evaluate patient care to achieve prioritized patient outcomes according to LPN scope of practice.
- Obtains admission assessment, including vital signs, medication and allergies.
- Performs patient/family teaching for assessed knowledge deficits within LPN scope.
- Demonstrates personal accountability in the delivery of patient care in a competent, professional and compassionate manner.
- Demonstrates organizational skills in handling multiple patients while assisting the health care provider.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Monitor patients for changes in mental or physical condition and promptly report concerns to the care team.
2. Performs and maintains documentation.
- Documents accurate patient information that reflects nursing interventions, patient responses, patient teaching and status of outcomes at discharge.
3. Communicates effectively.
- Communicates and collaborates effectively with other health team members regarding patient condition and needs,
- Coordinates ordered ancillary services and previous test results to meet specialist physician information needs.
- Coordinates closely with Psychiatric Providers to support medication management, including refills, patient education, and adherence monitoring.
4. Promotes and ensures patient safety in performance of all responsibilities.
- Administers medications, treatments and procedures in a timely and safe manner according to physician orders and nursing policy, including appropriate performance of standard precautions, using two patient identifiers, and performing patient risk assessments every shift.
- Demonstrates proficient technical/clinical skills and operational knowledge of equipment in Clinic.
- Informs and involves manager regarding patient care issues in a timely and appropriate manner.
- Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to patient and injury to self.
- Participate in crisis intervention and de-escalation under established protocols.
5. Follows HIPAA regulations and ensures that the confidentiality of patients medical, personal, financial records, employee records, and organizational records is maintained.
6. Participates in FCH committees, quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.
7. Complies with the state and federal regulatory requirements related to the performance of FCH operations and requirements of the FCH Compliance Program. Including but not limited to:
- The Compliance Policy.
- Completes all on-line training annually and as assigned.
- All department and organization-wide policies.
- Considers the impact of regulations such as Critical Access Hospital requirements, along with Federal and State of Nebraska regulations and discusses concerns appropriately to ensure compliance.
8. Presents self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
