Key Responsibilities:

Perform comprehensive assessments to develop individualized care plans that align with hospice philosophy.

Provide direct nursing care including pain and symptom management, medication administration, and comfort care.

Educate and support patients, families, and caregivers on disease process, medications, and end-of-life expectations.

Serve as case manager, coordinating with physicians, social workers, chaplains, aides, and the interdisciplinary hospice team.
