Home Health RN- Admissions 7 days on/7 days off

We are looking for a compassionate and skilled Registered Nurse (RN) to join our team as an Admissions Nurse. In this role, you will assess potential patients for admission, provide nursing care, and develop care plans. You will work closely with the intake team, referring physicians, and families to ensure the best care for our patients.

Key Responsibilities:

• Assess patients and families for admission into VNA programs.
• Develop and initiate care plans based on assessments.
• Provide direct nursing care and health education to patients and families.
• Collaborate with interdisciplinary teams to ensure seamless care coordination.

Qualifications:

• Graduate of an accredited nursing school (Bachelor’s Degree preferred).
• Current RN licensure in Missouri and Kansas.
• 1-2 years of acute care experience and home health/hospice experience preferred.
• CPR certification and valid driver's license.
