Description

  • Assessment and Planning:

    Conduct thorough psychosocial assessments of residents to understand their needs and develop individualized care plans. 

  • Advocacy:

    Advocate for residents' rights and dignity, ensuring their needs are met and their preferences respected. 

  • Family Support:

    Provide counseling and education to families, helping them navigate the healthcare system and understand care options. 

  • Discharge Planning:

    Coordinate discharge planning for residents, ensuring they receive appropriate support upon leaving the facility. 

  • Collaboration:

    Work collaboratively with healthcare professionals, staff, and community resources to provide comprehensive care. 

  • Communication:

    Facilitate communication among residents, families, and staff, ensuring everyone is informed and involved in care decisions. 

  • Crisis Intervention:

    Provide support and interventions during crisis situations for residents and families. 

  • Quality Improvement:

    Participate in quality improvement initiatives to enhance the overall quality of care. 

Qualifications

  • Must Be Current License Social Worker for State of Texas
  • 2 Years of admission/discharge experience
  • At least one year of Long Term care, Case worker or Hospital experience
