Position Overview: RN Case Manager
ABOUT THE JOB
The RN hired for this position will focus either on Hospice Case Management. The RN case manager will provide support to the patient's family as they learn to care for their loved one at home. The Hospice RN must be capable of compassionate communication with patients and their families while remaining vigilant in quality patient care. He/she should also have the emotional and physical stability to provide real-time solutions to the challenges of severe illness and death. He/she will also be responsible for patient assessments and creating/ adjusting plan of care needs as assessed during shifts. The RN will work closely with our Palliative Care service line.

This position reports to Director of Nursing / DON
Environmental and Working Conditions
Works in patient home and skilled nursing facilities in various conditions; ability to work flexible schedule; ability to travel within the Dallas/Fort Worth and surrounding counties;
Physical and Mental Effort
Ability to lift up to 50 lbs, to transfer and move patients. Requires working under some stressful conditions to meet deadlines of patient needs and charting guidelines. Quick decision and resource acquisition. Ability to meet patient/family individualized supportive service needs and coordinate cohesively with other care team members to best serve our patients and families. Requires hand-eye coordination and manual dexterity.
