Position Overview: Senior PCA

The Patient Care Advocate/Sales Representative plays a critical role in increasing access to hospice services by educating healthcare providers, patients, and families about the benefits of hospice care. This individual will be responsible for building and maintaining strong referral relationships, driving patient referrals, and representing the organization in the community. The ideal candidate combines compassionate communication skills with a results-driven mindset to help ensure individuals receive timely, high-quality end-of-life care.

Key Responsibilities:

1. Business Development:

· Develop and execute strategic sales plans to identify and engage referral sources, including physicians, hospitals, nursing homes, assisted living facilities, and community organizations.

· Build and maintain relationships with key healthcare providers, discharge planners, and social workers, to drive patient referrals.

· Meet or exceed sales goals and targets for patient admissions and referral growth.

2. Education and Outreach:

· Educate referral sources, patients, and families on the benefits of hospice care and the services provided.

· Provide in-service presentations, lunch-and-learns, and community seminars to increase awareness of hospice care.

· Stay informed about industry trends, regulatory changes, and competitive services to position the organization effectively.

3. Patient Advocacy:

· Work with patients and families referred to hospice care to explain services, address concerns, and facilitate the enrollment process.

· Ensure a seamless transition to care by coordinating with the hospice clinical team and referral sources.

4. Collaboration and Reporting:

· Collaborate with the clinical team to ensure patient needs are met and to provide feedback on service delivery.

· Track and report referral and sales activity.

· Regularly analyze sales performance data to identify opportunities for growth and improvement.

Qualifications

Education: Bachelor’s degree in business, marketing, healthcare, or a related field (preferred).

Experience:

· Proven success in healthcare sales, preferably in hospice, palliative care, home health, or a related field.

· Strong understanding of home health services, Medicare/Medicaid regulations, and end-of-life care.

Skills:

· Excellent communication, negotiation, and relationship-building skills.

· Ability to handle sensitive conversations with empathy and professionalism.

· Strong organizational and time management skills.

Preferred Qualifications:

· Experience in working with physicians, hospitals, and long-term care facilities.

· Passion for or prior experience in home health services.

Work Environment:

· Primarily field-based with frequent travel to referral sources and patient facilities.

· Some evening or weekend hours may be required to meet with patients or attend community events.
